A DMV-349 crash report uses fields for each unit, driver, non-motorist, narrative, diagram, and contributing circumstance. The NCDMV instructional manual gives separate handling for pedestrians, pedalcyclists, and scooters.

Use terms to locate the record, not to draw a conclusion

  • Unit number and vehicle or non-motorist identity
  • Driver, occupant, owner, and insurance fields
  • Narrative, diagram, and contributing-circumstance entries
  • Supplemental pages or corrections when available

A report may be important but is not a complete determination of legal responsibility or damages. It can be one record considered in a Charlotte car-accident claim.
