An attention-change record should identify the task, baseline, observed problem or improvement, duration, errors or help, provider record, restriction, accommodation, and later reassessment.
Use the same task measures over time
- Reading, screen work, conversation, instruction-following, scheduling, calculation, equipment, driving, or another specific task
- Duration attempted, interruptions, errors, reminders, assistance, rest, completion, and later symptom response
- Provider history, examination or test, finding, diagnosis if made, restriction, treatment, referral, and reassessment
- Job duty, production or safety requirement, accommodation, modified work, missed time, wage record, and supervisor communication
Do not turn a self-test into a diagnosis
Use personal logs to describe function and clinical records to document professional findings. An easy day and a difficult day both belong in the chronology. Preserve prior relevant conditions and medication information as actually documented.
