A missed appointment or interruption in care does not answer whether a workplace injury exists or whether later treatment is related. It does create a period the medical and claim records may not explain. Authorization delays, provider availability, transportation, illness, work conflicts, caregiving, medication effects, or a changed treatment plan should be documented when they occur.
A treatment-gap record should identify the recommended care, authorization status, scheduled date, reason it did not occur, notice given, response, rescheduled date, symptoms during the gap, and any later medical explanation.
Start with the Commission file and authorized-care path
The North Carolina Industrial Commission’s forms page identifies Form 18, medical-travel Form 25T, additional-medical-compensation Form 18M, and other forms used for specific issues. A form label does not determine whether it applies; record the claim number, injury date, employer, carrier or administrator, and current acceptance or dispute status.
Track every treatment step
- Referral, order, prescription, test, therapy, procedure, equipment, medication, or follow-up and the provider who recommended it
- Authorization request, recipient, date, records supplied, response, conditions, denial reason, and escalation channel
- Appointment offers, accepted date, cancellation, transportation, interpreter, work schedule, and rescheduling attempts
- Symptoms, function, medication effects, restrictions, work status, and material changes reported to the provider
G.S. 97-25 addresses medical compensation and specified employer and Commission roles. Treatment selection, change-of-physician questions, emergency care, and disputes require attention to the actual claim status and current Commission procedure rather than a general assumption that any provider visit will be covered.
Document a gap without rewriting the chronology
Keep the original missed or canceled appointment entry. Add the reason, supporting message, person notified, and new date. If the employee stopped because symptoms improved and later returned, record that sequence accurately. If the provider changed the plan or the carrier had not authorized it, preserve the written instruction or decision.

- Job duties, schedule, physical and cognitive demands, equipment, travel, and safety-sensitive tasks
- Work notes, restrictions, offered duties, wage statements, missed time, reduced hours, leave used, and return attempts
- Symptoms during modified duty, notice to the employer and provider, revised restrictions, and next review
