Critical care does not always produce a final answer at the same time as an order, test, image, procedure, consultation, transfer, or discharge. Some information remains preliminary; some reports are signed later; some recommendations depend on another result. A pending-findings register keeps unresolved items visible without guessing what they mean.

A pending-findings register should show the order or question, responsible professional or facility, expected source, status, result date, communication, follow-up, and the exact point when provisional information changed.

Enter each unresolved item at the time it arises

  • Date and time ordered, requested, collected, performed, discussed, transferred, or identified as missing
  • Test, imaging study, pathology, procedure report, consultation, culture, monitoring result, diagnosis question, referral, restriction, or follow-up item
  • Ordering professional, performing department or facility, consulting service, records system, receiving professional, and contact channel
  • Clinical question or purpose exactly as documented, without adding a diagnosis, cause, prognosis, or treatment recommendation
  • Expected report or next step, estimated availability if supplied, person assigned to check, next review date, and escalation contact

Preserve preliminary, amended, and final versions

A portal notification, preliminary imaging interpretation, operative note, laboratory value, verbal update, signed report, addendum, and later consulting-clinician assessment may be separate records. Keep each source with its author, service date, entry time, signature time, version, and stated status. Do not replace an earlier record when a later entry changes or qualifies it.

HHS explains that, with exceptions, an individual may inspect, review, and receive copies of medical and billing records held by covered providers and health plans. Request the facility, department, encounter, date range, record category, image or data when applicable, and later addenda rather than relying only on a portal summary.

45 C.F.R. § 164.526 addresses amendment requests for protected health information. Preserve the disputed entry, request, response, accepted amendment, denial, statement of disagreement, and later disclosure history. The register should show the correction trail rather than silently revising the source.

Carry unresolved items through every handoff

The Centers for Medicare & Medicaid Services provides a discharge-planning checklist that prompts patients and caregivers to address medicines, equipment, appointments, care tasks, warning signs, and who to call with a problem. For every pending item, identify what the departing setting communicated, what the receiving setting received, and who owns the next check.

  • Pending item listed in the transfer or discharge material, omitted, or communicated separately
  • Receiving professional or facility, date and method sent, confirmation received, and record of review
  • Appointment, repeat test, referral, monitoring, equipment, medication, restriction, or other instruction linked to the item
  • Patient or representative question, professional notified, response, new instruction, and updated source
  • Closed date, final document, unresolved exception, and next responsible person

Separate observation from qualified interpretation

A patient or family member can record that a result was pending, a call occurred, a report appeared, an appointment was scheduled, a task was completed, or an observable function changed. Diagnosis, medical causation, prognosis, permanence, and the significance of a result require an appropriate professional foundation. North Carolina Rule of Evidence 702 addresses qualified expert testimony; a tracking sheet does not create that opinion.

Close the register only from a dated source

  • Final report received and matched to the order, encounter, person, body part, specimen, study, or procedure
  • Result or recommendation communicated by an identified professional, with the date, recipient, and next instruction recorded
  • Follow-up completed, declined, cancelled, transferred, or still unresolved, with the supporting note or communication
  • Earlier assumption, provisional label, or incorrect identifier preserved with the later correction and reason
