A report and a form can create a record of a work-related injury, but they do not decide whether a claim will be accepted or what benefits may be available.

Record the report of injury

  • The date, time, place, activity, body area, and people who may have observed the event
  • The date and method used to notify the employer, the person notified, and any response received
  • Medical-provider information, work restrictions, lost-time records, and copies of communications
  • A copy of each completed form and proof of submission or delivery

The North Carolina Industrial Commission’s forms page lists current workers’ compensation forms and filing resources. Instructions and deadlines can depend on the facts, so a general guide should not be treated as a determination of eligibility.
