Availability, service area, hours, eligibility, documents, wait lists, cost, and capacity can change, so record the specific referral and confirm it directly with the provider.

  • Person reached, information supplied, consent, reference number, appointment or request, response, denial or delay reason, appeal or review path if stated, and alternative

Keep health care and record access in separate lanes

Medical questions and care decisions belong with qualified health-care providers. Administrative support may involve scheduling, transportation, pharmacy access, billing, benefits, or obtaining records, but it should not substitute for diagnosis or treatment.
