A break between medical visits may reflect recovery, scheduling, referral delay, transportation, cost, insurance authorization, work, caregiving, relocation, or a decision not to pursue recommended care. A claim file should document the actual reason rather than fill the interval with an assumption.
A treatment-gap record is useful when it identifies the dates, recommended next step, reason for delay, efforts made, symptoms and function during the interval, and what happened when care resumed.
Create a gap-specific timeline
- Last visit before the interval and the provider’s documented plan
- Referral, scheduling, authorization, transportation, language, work, financial, or caregiving communications
- Symptoms, medication, home instructions, activity limits, work status, and material changes during the interval
- First later visit, history given, examination, diagnosis, treatment, and follow-up plan
Keep explanation separate from clinical opinion
Preserve appointment confirmations, cancellation messages, referral notes, insurer authorization records, bills, and transportation receipts. A personal statement can explain practical circumstances, but only the treating record shows what was reported and what the provider concluded. If a record is inaccurate, use the provider’s correction process rather than rewriting the note.
