Prioritize safety and instructions

Record physical conditions without interpretation
- Date, time, exact location, direction of travel, lane layout, traffic controls, weather, lighting, surface condition, and visibility
- Vehicle positions, visible damage, debris, cargo condition, marks, lights, photographs, video, and the person or device that created each record
- Names and contact information of participants and witnesses, with personal observations marked separately from statements made by others
- Report number, responding agencies, towing, storage, and conditions that changed before the note could be made
Preserve unknowns as questions
A scene can change rapidly. Mark missing information, uncertain timing, and items not visible rather than filling gaps with a conclusion. Keep original files and record when each item was created or received.
