Terms can have a limited statutory meaning

North Carolina G.S. 90-21.11 contains definitions used in Article 1B, including health care provider and health care institution. Read the current definition with the surrounding statute and the document at issue; a label in a record does not independently resolve a legal question.

Preserve identifying information

  • The provider or facility name, role, location, date, and the document where the term appears
  • The original record, billing material, communication, or notice that uses the term
  • A dated list of records requested, received, and missing
  • Questions about terminology kept separate from conclusions about care or responsibility
